Stroke Personal Record for health professionals

Our Personal Stroke Record is a patient-facing template in the Welsh language. It is for healthcare professionals only to complete and give to newly-diagnosed stroke survivors in hospitals.

Please note: For orders of over 20 items, the first 20 will be free of charge. You will then be charged £0.20 for each additional item. The cost of the first 20 will be removed at the checkout.
