The Stroke Support Pack consists of a ‘hello’ folder which introduces our organisation. It lets people know who we are and how we are here to support them throughout their recovery journey – whenever they need us.
It also consists of two guides, a guide for people who have had a stroke and a guide for people who are supporting someone after a stroke. These are called:
The guides contain useful information to support recent stroke survivors to:
- understand what has happened and how it might affect them.
- give answers to common questions about stroke. Including questions around recovery and rehabilitation, secondary prevention and daily living after returning home.
- let them know they do not have to face recovery alone.
- find out how the Stroke Association can support them and where to find further help and further information.
Please order for and share the pack with your patients before discharge.
Recovery in hospital is just the start of someone’s stroke journey. Together we can make sure no one faces life after stroke alone.
